If someone asks you to fill a similar form, then you have to be careful. This kind of information is not asked from you by IRCTC.
IRCTC itself has tweeted that, “Such people call from different numbers and send some links to the customers. Please do not respond to such links or calls. IRCTC has told that the entire refund process is automatic. There is no human intervention involved in the refund process of IRCTC.
