The thing is, hypertension (high blood pressure) is often symptomless—it’s the silent killer—so the ability to flag it opportunistically is the major shift here.

Starting today, Apple Watch users in India can activate a feature designed to alert them if the device detects signs of chronic high blood pressure (hypertension).

How the Detection Works

This isn’t a replacement for a medical blood pressure cuff, or nothing. It works completely passively in the background, using data that the watch already collects.

  • The Sensor: It uses data from the optical heart sensor—the same green light LEDs on the back of the watch.

  • The Algorithm: It analyzes how a user’s blood vessels are responding to the beats of the heart. The logic is that consistent hypertension stiffens blood vessels, and the watch’s algorithm detects that change over time.

  • The Timeline: The algorithm doesn’t check just once. It reviews the background data consistently over 30-day periods. This focus on consistency helps avoid false alarms from temporary spikes due to stress or exertion.

The Problem It Solves

Hypertension is the leading modifiable risk factor for severe health events like heart attack, stroke, and kidney disease.

  • Undiagnosed: Approximately 1.3 billion adults globally are impacted, but it’s frequently undiagnosed because it has no obvious symptoms. Many people don’t see a doctor regularly, and even a single reading in a clinic can miss it.

  • The Reach: The feature is expected to flag over 1 million people with undiagnosed hypertension within the first year, which democratizes access to early detection.

What Users Need to Do

  1. Setup: Users must manually set up the Hypertension Notifications within the Health app on their iPhone 17 Pro.

  2. Notification: If the watch detects consistent signs of hypertension, the user gets an alert shown in the Health app.

  3. The Next Step (Crucial): If you receive a notification, it is recommended that you log your blood pressure for seven days using a third-party blood pressure cuff. And then you take those results to your doctor at your next visit. This aligns with the latest American Heart Association guidelines.

This is a serious move into opportunistic detection. It’s not diagnosing you, but it’s forcing a conversation between you and your provider.
